Member will sign the AF Form 108 acknowledging BE WELL Program enrollment, accepting responsibility for improving their fitness level, completing program requirement and if appropriate providing documentation of program compliance. Unit Commander or equivalent will use the AF Form 108 as a tool to document mandatory education and intervention requirements. The failure of command or command representatives to sign, annotate, or otherwise complete the AF Form 108 in no way lessesns the member’s overarching responsibility for his/her own fitness and compliance with AF Fitness standards.
